講座概要

神経学は、解剖学と症候学に基づく論理的なサイエンスです。診察によって得られた神経学的所見から責任病変の局在診断を行い、系統的かつ的確な病歴聴取から臨床診断を行います。かつて脳は『人類最後のフロンティア』と呼ばれ、多くの脳神経疾患の治療は諦観されてきました。しかし“Decade of the Brain”に象徴される脳科学推進の世界的潮流が、病態解明と診断、治療開発の強い追い風になっています。高齢化により増加傾向にある脳神経疾患に対して、私たちは探求と篤学の士となり、知識をアップデイトしながら教育、診療、研究に取り組んでいます。

特徴的な臨床の取り組み

当科では、精緻な神経診察と丁寧な病歴聴取を基盤に、脳卒中をはじめとする急性神経疾患から、パーキンソン病や認知症などの神経変性疾患まで、幅広い脳神経疾患の診療に取り組んでいます。脳卒中診療では、迅速な病型・病態診断と急性期治療を行うとともに、再発予防や後遺症への対応、その後の生活を見据えた継続的な診療を重視しています。一方、神経変性疾患では、患者を診察時点の症候だけでなく、発症から進行期に至る時間軸のなかで捉え、運動・認知・自律神経機能、身体的フレイル、生活機能を包括的に評価しています。また、地域の医療機関や多職種と連携しながら、急性期から慢性期まで切れ目のない医療を提供するとともに、日常診療から生まれた問いを画像・バイオマーカー・臨床研究へと発展させ、その成果を診断、予後予測、治療へ還元することを目指しています。

岩手医科大学脳神経内科・老年科は本学に神経学の教室が開設されて以来40年以上にわたり、岩手県における脳神経疾患診療の中核を担ってきました。脳卒中、認知症、パーキンソン病、神経免疫疾患、頭痛をはじめとする幅広い疾患を対象に、診療、教育、研究に力を注いでいます。

2019年の附属病院の矢巾町移転を機に、診療科名を「脳神経内科・老年科」とし、2020年度からは講座名も「内科学講座脳神経内科・老年科分野」へ変更しました。精緻な神経診察を基盤とする伝統的な神経学を大切にしながら、超高齢社会の要請に応える新しい脳神経内科学・老年医学を実践することが、私たちの使命です。

教育は、当科が長年にわたり大切にしてきた柱の一つです。学生教育では、系統講義を通じて神経学を包括的かつ効率的に学べるよう工夫するとともに、臨床実習では実際の神経症候に触れ、診察から局在診断、臨床診断へ至る思考過程を「体得する」ことを目標としています。一人でも多くの学生が神経学の論理性と奥深さに魅力を感じ、その門を叩いてくれることを願っています。若手医師の育成においては、神経電気生理学、神経病理学、神経画像診断学などを体系的に学べる指導体制を整え、日本神経学会認定神経内科専門医の育成に力を入れています。また、総合内科専門医、脳卒中専門医、脳神経血管内治療専門医、認知症専門医、頭痛専門医など、各分野の専門医および指導医のもとで研鑽を積むことができます。県内の脳神経内科医の多くが当科および関連施設で診療に従事し、岩手の脳神経内科医療を支えていることは、私たちの誇りです。

診療面では、脳神経疾患に関する高度で専門的な医療を提供しています。一刻を争う脳卒中の急性期診療から、認知症やパーキンソン病など長期的な支援を必要とする神経変性疾患まで、患者さんを時間軸のなかで捉え、急性期から慢性期まで切れ目なく支えることを重視しています。同時に、県立病院をはじめとする県内各地の基幹病院へ医師を派遣し、広大な岩手県の地域医療、とりわけ医療資源の限られた地域の脳神経内科診療を支えています。超高齢社会が現実となった現在、脳神経疾患の専門診療と老年医学的な視点を併せ持つ当科の役割は、ますます重要になっています。

研究面では、脳卒中、認知症疾患、パーキンソン病と関連疾患、神経免疫疾患、頭痛疾患などの研究グループを設け、定期的な勉強会やカンファレンスを通じて研究計画を検討し、学会発表や論文発表へつなげています。なかでも、矢巾町と連携して進めている前向きコホート研究である“YAHABA study”には講座を挙げて取り組んでおり、地域に根差した研究から新たなエビデンスを世界へ発信することを目指しています。大学院生をはじめ、臨床から生まれた問いを深く追究したい若い研究者にも、広く門戸を開いています。古典的な神経学の眼を大切にしながら、脳神経科学の新たな地平を切り拓く。東北・岩手の地から、これからの脳神経内科医療をともにつくる意欲ある皆さんを、心から歓迎します。

講座における主たる研究テーマ

脳卒中グループ

入院データベースから急性期脳卒中、矢巾コホートから無症候性脳血管障害を対象とし、画像のみならず脳神経超音波を駆使した臨床研究を行っています。

認知症グループ

矢巾町高齢者の認知症前向きコホート研究（YAHABA study）を中心とした認知症研究を行っています。日本の地域高齢者の健康維持が重要と考えています。

パーキンソングループ

発症前のバイオマーカー開発や発症早期の診断と運動症状の治療、非運動症状のQOL改善、運動合併症の管理、生の尊厳、全てのステージを研究対象にしています。

神経免疫疾患グループ

多発性硬化症を中心に免疫関連疾患のADLとQOLにコミットする研究を行っています。

頭痛グループ

慢性頭痛患者さんの心理的要因と日常生活支障度に注目した研究を行うと共に、頭痛診療医の啓発と教育に力を入れています。
