講座概要

岩手医科大学内科学講座循環器内科分野は、約80名の医局員を擁し、虚血性心疾患、不整脈、心不全、弁膜症、心筋症、大動脈・末梢血管疾患など、循環器領域全般を幅広く診療する講座です。大学病院における高度で先進的な循環器医療の提供と、岩手県を中心とした北東北各地域の医療を支えることを、重要な使命としています。

大学病院では、循環器救急や重症心不全に対する集学的治療に加え、冠動脈インターベンション、不整脈アブレーション、TAVIをはじめとする構造的心疾患へのカテーテル治療、大動脈・末梢血管治療など、国内最高水準の循環器診療を展開しています。各専門分野が緊密に連携し、患者さん一人ひとりに最適な医療を提供するとともに、県内外の医療機関との強固なネットワークを活かし、どの地域においても質の高い循環器医療を受けられる体制づくりに取り組んでいます。

また、日々の臨床から生まれる疑問を新たな医学的知見へとつなげるため、基礎研究から臨床研究、多施設共同研究まで、循環器医療の発展に資する研究を積極的に推進しています。国内外の研究機関や医療施設との交流・連携を深め、岩手・北東北から全国、そして世界へ向けて、新しいエビデンスを発信することを目指しています。

高度な臨床、地域医療、研究、教育を一体として推進し、次代の循環器医療を担う医師を育成することも、私たちの重要な役割です。地域に根ざしながら国内外に広く開かれた講座として、循環器医療のさらなる発展と、地域の皆さまの健康に貢献してまいります。

特徴的な臨床の取り組み

① 国内有数の実績を誇る、構造的心疾患のカテーテル治療

当科は、構造的心疾患（Structural Heart Disease：SHD）に対するカテーテル治療において、国内有数の治療実績を有する施設です。大動脈弁狭窄症に対する経カテーテル大動脈弁留置術（TAVI）をはじめ、僧帽弁・三尖弁疾患に対するカテーテル治療、左心耳閉鎖術、心房中隔欠損症に対する閉鎖術など、日本で行われている幅広い低侵襲治療に対応しています。各治療において全国でも上位に位置する豊富な症例経験を重ね、安定した良好な治療成績を維持しています。豊富な経験をもとに、患者さん一人ひとりの心臓や血管の形態、全身状態、生活背景まで詳細に評価し、最適な治療方法を検討しています。標準的な症例だけでなく、複雑な解剖や複数の疾患を併せ持つ治療難度の高い症例にも、蓄積された知識と高度な技術を活かして対応しています。

② 新しい診断技術を積極的に取り入れた、精密な循環器診療

循環器医療では、病気を正確に診断し、一人ひとりに適した治療法を選択することが重要です。当科では、高精度な心臓画像診断や生理学的評価、三次元画像解析など、進歩を続ける新しい診断技術を積極的に導入しています。得られた情報を治療前の詳細な評価やシミュレーションに活用し、治療の必要性、適切な時期、最適な治療方法を多角的に検討することで、より安全で精度の高い循環器診療を目指しています。

③ 重症循環器疾患に対応する集学的診療体制

急性心筋梗塞、心原性ショック、重症心不全、致死性不整脈など、一刻を争う循環器疾患に迅速に対応しています。最大24床の集中治療室（ICU）と、ハイブリッド手術室を含む複数の心臓カテーテル治療室を備え、必要に応じて補助循環装置や最新の治療技術を活用します。救急搬送からカテーテル治療、集中治療、回復期のリハビリテーションまで、一貫した診療を提供できる体制を整えています。

④ 広大な岩手県・北東北を支える循環器診療ネットワーク

岩手県は全国で2番目に広く、専門的な循環器医療へのアクセスには地域差があります。当科では、岩手県内および北東北の関連医療機関と緊密に連携し、地域での診断から大学病院への紹介・搬送、高度治療、治療後の地域でのフォローアップまで、切れ目のない診療体制の構築に取り組んでいます。

患者さんがどの地域に暮らしていても、必要なタイミングで適切な循環器医療につながることを目指しています。

⑤ 循環器医療の未来を切り拓く臨床研究

日々の診療から生まれる疑問や課題を、より良い診断・治療へとつなげるため、臨床研究を積極的に推進しています。大学病院内での研究に加え、東北地方や全国の医療機関と連携した多施設共同研究、新しい治療法や医療機器の有効性・安全性を検証する治験にも取り組んでいます。新たな治療技術を導入するだけでなく、その治療成績を科学的に検証し、新しい医学的知見として国内外へ発信することを重視しています。東北地方における臨床研究の拠点として、国内の循環器医療の発展をリードできる環境づくりを進めています。

⑥ 次世代の循環器医療を担う医師を育てる教育

高度な循環器医療を将来にわたって地域へ届けるためには、次世代を担う医師の育成が欠かせません。当科では、経験豊富な指導医のもと、診断から治療、術後管理までを段階的かつ体系的に学べる教育体制を整えています。特に、カテーテル治療を安全かつ確実に行うオペレーターの育成に力を入れ、技術だけでなく、治療適応の判断や合併症への対応を含めた総合的な能力の習得を目指しています。また、関連病院で地域医療に従事している医師も、大学病院の最新の診断・治療や臨床研究を継続的に学べる環境づくりを進めています。

また、医学部学生に対しても、循環器診療の魅力と社会的役割を実感できる、分かりやすく実践的な教育を重視しています。教育を通じて診療の質を高め、将来にわたって地域の患者さんに高度な循環器医療を提供できる体制を築いていきます。

教授挨拶

虚血性心疾患、不整脈疾患、末梢・中枢動静脈疾患、心不全、弁膜症、心筋症、感染性心内膜炎などのあらゆる循環器診療を担当致します。初期の研修期間はすべての分野を網羅できる医師を育成すべく、人材教育、各人のキャリアを意識した教育プログラムを準備しております。岩手県内、隣県に関連病院を多数もち、臨床経験も豊富に積むことができますし、明確な目標のある方には海外留学を積極的に奨励して参ります。家庭の事情などにも最大限配慮し、各人にあったフレキシブルな教室運営をしていくことが医局の共通のルールとしています。せっかく積んだ医師のキャリアを簡単に途切れさせないように、せっかく一緒に勉強を始めた仲間が長く働ける様な環境を整備することが、医局の責務と考えております。

講座における主たる研究テーマ

本講座では、「日常診療で生じる臨床課題を研究によって解決し、循環器診療の質を向上させること」を基本理念として、虚血性心疾患、構造的心疾患（Structural Heart Disease: SHD）、不整脈、心不全を主要な研究領域としています。

虚血性心疾患領域では、新規薬剤溶出ステントや薬剤コーティングバルーンなどの治験・臨床研究に積極的に参画するとともに、岩手県全域を対象とした急性心筋梗塞患者レジストリーを構築し、ガイドライン推奨薬物治療（GDMT）の実施状況と長期予後を検証しています。さらに、冠動脈CTによるプラーク解析、CT-FFRやAngio-FFRなどの最先端画像診断技術を臨床へ導入し、AI・機械学習を活用した冠動脈疾患の発症・予後予測モデルの構築など、次世代の循環器診療を見据えた研究を推進しています。

SHD領域では、TAVIを中心に、世界的臨床試験であるPARTNER試験をはじめとした国際共同研究へ参加する一方、日々の診療から生じる新たな臨床課題に着目した研究を特色としています。技術革新とともに新たに顕在化する課題に対し、CT・心エコー・病理・血行動態評価を融合した多角的解析を行い、診断・治療戦略の最適化を目指しています。また、新規デバイス導入時の安全性・有効性の検証にも積極的に取り組み、得られた知見を国内外へ発信しています。

心不全領域では、心筋症や心膜疾患など比較的稀少な疾患から、慢性心不全の日常診療まで幅広いテーマを対象に、多施設共同研究を展開しています。リバースリモデリングや薬物療法最適化、心筋障害の病態解明などを通じ、新たな診断・治療戦略の確立を目指しています。

不整脈領域では、パルスフィールドアブレーションやBrugada症候群などを対象とした多施設共同研究に参画し、新規治療技術や診療実態の評価を通じてエビデンス創出に貢献しています。

本講座は、基礎・臨床・画像診断・AI技術を融合した研究を推進するとともに、多施設共同研究や国際共同研究を積極的に活用し、岩手から世界へ新たなエビデンスを発信することを目標としている。
